- State-certified birth certificate
- Immunization record (View state immunization requirements)
- Proof of residency (utility bill, lease agreement, etc.)
- Register your child at their designated boundary school. School offices open on Aug. 14.

Your child’s elementary school will provide most classroom supplies for the 2018-19 school year. Families of elementary students are asked to provide only the following items:
- 1 set of earbuds (headphones)
- 1 backpack
- 1 box of quart-size baggies or 1 box of gallon-size baggies (to be determined by each teacher)

Families must provide the following for their child to participate in middle school and high school athletics
- Student physical form (must be completed annually)
- Student fees (ASB and sport participation)
- Proof of insurance

Middle school and high school students may qualify for a Ben Franklin Transit bus pass provided by the transportation department. Learn more about the program and if your child qualifies.

Free/reduced applications for the 2018-19 school year will be mailed to all district households the first weekend of August. The online application will be available August 7. You will need your student’s 8-digit ID number for the online application.
Last year’s eligibility is good through Oct. 9, or until a new application is received. Applications are processed in the order in which they are received. Parents are encouraged to submit applications early. Applications received after Aug. 23 may not be processed by the first day of school.
Some families have been automatically qualified for free or reduced meals already through the direct certification process. If you receive a notification letter saying your child has automatically qualified, then you do not need to submit an application. Please check the letter carefully to make sure all your children are listed.
The following schools will provide free meals to all their students as part of the district's participation in the federally-funded Community Eligibility Provision. Families with children at those schools do not need to apply for free or reduced price meals unless they also have students at non-CEP schools.

Families may download the So Happy app for their smart phone for free to have quick access to menus at their child's school, including nutritional information and potential food allergens. That information is also available on the Sodexo website.
