If you qualify for free or reduced-price meals, you may also be eligible for waived/reduced fees to participate in other school programs. Submitting/not submitting this form will not affect your child’s eligibility for free or reduced-price meals.
If you give consent, Nutrition Services will share your student’s eligibility information with the appropriate school personnel to facilitate participation and waived/reduced fees.
To give consent you must check the box for each program you would like to participate in, write your student’s name, sign, and date.
This form allows Nutrition Services to notify the school of your student’s eligibility. The school provides the fee waivers/reduction.
