Forms
PDF forms are very difficult to make accessible. We recommend, if possible, converting your PDF form into a web-based form or using Google Forms.
Google Forms can be used to make surveys, sign-ups, and questionnaires that meet digital accessibility standards and provide equitable access for people with disabilities.
Theme and Header
- Title
- Give the survey a meaningful title and description in the header
- Update the file name to match the title
- Header
- Use no image, a simple image, or an image that provides relevant context for the form
- Theme
- Use the standard theme colors and fonts
- If changing colors, check for good contrast on the ‘Submit’ and ‘Next’ buttons
- If changing fonts, choose legible fonts no smaller than size 12
Questions
- Instructions
- Provide clear prompts in each question and clear labels for choices
- If needed, provide detailed question instructions by adding description (More options > Description)
- If needed, provide general instructions at beginning of survey
- Required
- Don’t make questions required unless absolutely necessary
- Images
- If including images, provide meaningful description in the image caption (caption will also be used as alt text)
- Long surveys
- Break survey into sections with headings by using a Title and Description block
More Considerations
- Settings
- Review survey settings and adjust as needed
- Toggle on ‘Show Progress bar’ feature.
- Readability
- Use appropriate language for your intended audience
- Use title case for headings and sentence case for everything else
- Video
- If using video, make sure the video is captioned and that the captions are correct
