Nutrition Services

If your student has a medically-certified disability that requires a change or substitution to the posted menu, please fill out the Request For Special Dietary Accommodations form and provide to your school nurse.  The school nurse and RSD Director of Nutrition Services will review your form to make accommodations. A copy of this form will be kept with the school nurse, school kitchen and at the Nutrition Service Department office.  The school nurse may route it further if needed. 

All Request for Special Dietary Accommodation forms must include the signature of a state-recognized medical authority (state-licensed health care professionals authorized to write medical prescriptions under state law). These include health professionals with one of the following credentials:

• Medical Doctor (MD)
• Doctor of Osteopathy (DO)
• Physician’s Assistant (PA) with prescriptive authority
• Naturopathic Physician
• Advanced Registered Nurse Practitioner (ARNP)

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
