Student Profile Forms

Student profile forms for the 2016-2017 school year are available in the office and are due by 4/29/16.  These forms are for parents who have specific things that they would like the placement team to consider before deciding on a classroom for next year.  These would include social/emotional, learning environment, health or behavior issues.  We do NOT take requests for specific teachers.  Our placement teams are very careful to place students in an optimal learning environment.
